Waitlist and contact forms: When you submit a waitlist or contact form, we collect the name, email, phone number, branch of service, and base information you provide. This information is used solely to contact you about Make Tape services.
Clinical information: If you become a patient, we collect medical-history information relevant to determining candidacy for the BCA Ready Protocol. Protected Health Information (PHI) is maintained under HIPAA and stored separately from marketing data.
Website analytics: We may use privacy-respecting analytics to understand how visitors use maketape.fit. We do not sell browsing data or use it for behavioral advertising targeting.
How We Use Information
We use information you provide to respond to your inquiry, schedule appointments, provide clinical services, and send you information you have requested (such as an opening-day discount code). We do not sell, rent, or trade your information to third parties.
